Arizona Complete Health – My Healthy Pays
Earn rewards for completing the following appointments:
$25 – Flu Shot (Ages 6 months+)
$25 – Infant and Well Child Visits (Ages 0-15 months that have completed at least 6 well child visits)
$25 – Child and Adolescent Well Child Visit (Ages 3-21)
$25 – Preventive Dental Visit (Ages 0-21)
$25 – HbA1C Test (Ages 18+, only for eligible diabetic members)
BCBS Health Choice – Health Rewards Program
Earn rewards for completing the following appointments:
$150 in a calendar year – Well Child Visits, $25 for each visit (Ages 0-15 months, 1 appointment ages 16-30 months)
